Man, 61 y.o.

Pathologic left cervical lymphatic node enlargement.
In the image, roundish node
(mm 15x18x18, 2,5 cc); the structure is hypoechoic, dishomogeneous, and there is no evidence of the hilum.
Large, very firm nodule in the left lobe
(cm 5x7x8 circa, 140 cc).
The trachea was displaced to the right side.
 

Serum calcitonin: 618 pg/ml (n. v. <10).

FNAB in the thyroid nodule: medullary carcinoma.
FNAB in a lymphatic node: medullary carcinoma.
Histological examination: medullary carcinoma.
